Upon industry requests, MDOT has recently updated their Materials Source List Form (Form 0501) and their Extension of Contract Time Form (Form 1100A) to be more user friendly by upgrading text boxes to allow for wrap text/multiple text lines. If you work with either of these forms, it may be beneficial to verify you are using the most up to date form.
