ALERT – Job Site Poster Deficiencies Noted

MDOT has informed MITA that during their recent field reviews of MDOT federal aid projects, the Federal Highway Administration (FHWA) has noted that some job site posters are lacking the required supplemental information.

Two posters in particular have caught the eye of the FHWA as lacking in the required supplemental information.

FHWA Form-1022 – Lacks the required “State Transportation Agency” and the “Federal Highway Administration Division Administrator” information in the boxes at the bottom of the form.

Form WH1321 – Lacks the required “Contracting Officer” information in the box at the bottom of the form.

We strongly encourage all MITA members to add this required supplemental information to these two identified forms at all of your job site poster locations.  This required supplemental information can be added with a sticker you create, or it can simply be hand-written on the poster.

We remind you that if you fail to add this required supplemental information to these posters, your Engineer could find you in noncompliance and initiate actions and potential penalties per the Special Provision for Jobsite Poster Deficiencies and Initial Mobilization Payment.
